妖精动漫 Educational Psychology Service - early intervention and prevention support offer

The 妖精动漫 Educational Psychology Service has developed a new early intervention and prevention support offer for education settings. The new service offer builds upon the temporary offer that has been in place since October 2024, which utilised one-off recovery funding to provide additional resources for the remainder of the 2024 to 25 academic year, as a proof of concept for the wide-ranging benefits of Educational Psychology (EP) involvement. You can read about this offer on the Educational psychology practitioner resources page.

Provision mapping

Provision mapping guidance for teachers, SENCOs and school leaders is available on the provision mapping page and within the Ordinarily available provision (schools) guidance.

You can also download the Provision Mapping template (DOC)

Education Health and Care forms

All Education, Health and Care (EHC) plans must be reviewed at least annually.

By law, it's the Local Authority's (LA) responsibility to complete the review process, but we delegate some steps to the school/setting/or other education provider, who are known as the host.

The Annual Review (AR) is more than just a meeting, it is a complete process, including actions that must be taken before and after the meeting. This process must be completed on or before the anniversary of when the EHC Plan was first issued or the anniversary of the last review. This is laid down in law and in the SEND Code of Practice (CoP).

For more information about the annual review process (including interim annual reviews), see the EHC plan statutory review process and documents.

Occasionally, children and young people's needs are so complex that a statutory assessment of their Education, Health and Care needs may be appropriate. There is no legal requirement for a school to spend a specific amount before initiating an assessment.

If you are a professional supporting a child or young person where you feel a statutory assessment may be required, please complete:

Once completed, please submit these to our new Education Health Needs and Assessment (EHCNA) team by emailing EHCNArequests@surreycc.gov.uk.

We recognise that schools are also using the Front Door/ for EHCNA requests. This route will remain open for now. For help using the online portal, refer to the Guide to using the Request for EHCNA online form (PDF). Please note, the online portal cannot accept zip (.zip) files. If you have additional documents that you cannot upload, email them to EHCNArequests@surreycc.gov.uk.

If you have any queries please contact EHCNAteam@surreycc.gov.uk.

妖精动漫 will consider information provided within this form when allocating assessment to Educational Psychologists. In some instances we will decide a virtual psychology assessment is appropriate.

Our team of multi-agency professionals will review your request and make a recommendation as to whether a statutory assessment is required, based on the child or young person's needs. We will always let you know the outcome within 6 weeks of the request being made.

There may be occasions that we do not agree that a statutory assessment is required, however, you will always be provided with the reasons for this and signpost you to services that may be able to offer additional advice and support.

For information about how we use the data, refer to Children, families, lifelong learning and culture privacy notice.
